Amergis Medical Social Worker in Oak Creek, Wisconsin
Pay: $55/hr. (reimburse gas receipts) - for LOCAL CANDIDATES
Pay: $2351 TOTAL weekly pay = $1000 taxable + $1351 stipends
Shift: Days, 8am - 5pm roughly
Schedule: Monday - Friday
Location: 6737 W Washington St, Milwaukee, WI 53214 is main address for travel
Setting: Hospice company, all patients will be hospice patients. Social Worker will be responsible for working as a Case Manager Social Worker. They will travel from patient to patient (LTC facilities, personal homes, etc.) and provide care as needed.
Skills / exp. required: 1+ year as a social worker, Masters degree, APSW or LCSW required, Hospice exp., working cark
Ratios: 4 or 5 visits a day
Territory: West Bend down to Highway 94
Minimum Requirements:
Baccalaureate degree in Social Work (BSW) or Master’s degree from a school of social work accredited by the Council on Social Work Education per state/client requirements required
Must have APSW or LCSW
Minimum of one (1) year of social work experience in a health care environment in the last two years preferred
Complies with all relevant professional standards of practice
