Connections Recovery Services is currently hiring for a Resource Coordinator position with our Mobile Crisis Response Team.

POSITIION DESCRIPTION

Position Title:  Resource Coordinator for Mobile Crisis Response Team Department: MCRT *Reports To: Executive Director and Lead Resource Coordinator *

Develops, plans, and implements strategies for program evaluation and growth. Organizes and coordinates post-intervention follow-up with individuals who engage with the Mobile Crisis Response Team. Supervises crisis response teams.

ESSENTIAL DUTIES AND RESPONSIBILIITES

This job description is not intended to be all-inclusive. Employee will also perform other reasonably related duties assigned by immediate supervisor.

• Provides oversight of the Mobile Crisis Response Team. • Engages in staff development and evaluation. • Ensures the documentation and quality of client specific and program data. Assists in maintaining the CIT Data repository. • Oversees orientation and training of staff members. • Manages staffing patterns and programmatic structure. • Provides in-the-moment supervision and consultation to staff working with individuals in crisis. • Response in person to a mental health crisis in the absence of/or in combination with the Case Manager and Intervention Specialist. • Organizes and coordinates post-intervention follow-up with individuals experiencing a mental health crisis. • Provides community resources and makes referrals, schedule follow-up services, navigate financial/insurance issues, and remove barriers to receiving services. • Collaborates with law enforcement and various mental health providers while maintaining positive relationships with community professionals. • Maintains a working knowledge of the mental health and human service systems in our region. • Completes all required charting, casework, documentation, and computer entry, on a timely basis. • Creates patient treatment plans; is a team player and must be willing to assist other team members. • Maintains a valid Ohio driver's license.

 

EDUCATION/EXPERIENCE/SKILLS

• Bachelor's degree in social work, psychology, or related field required. Masters preferred. • Independent license as a social worker, professional counselor or psychiatrist required. • Eligible for an NPI number and Ohio Medicaid number required. • Three years clinical experience preferred. • Holds a valid Ohio driver's license and pass a criminal background check.
