Description

Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York — plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.

Summary:

Under the general supervision of the rehab manager or supervisor, performs therapy examinations, develops treatment plans and goals, and applies interventions in accordance with the Standards of Practice of the profession, applicable state licensure regulations, and hospital policies and procedures to a variety of patients ranging from newborn to geriatric in age, in an outpatient care setting. Opportunity for rotation to acute care sites may exist per operational need.

Responsibilities:

  1. Performs therapy examinations, develops treatment plans, and establishes treatment goals with patient input utilizing evidence based practice and ASHA recommendations to guide care.

  2. Provides care to special populations including: Oncology, Stroke and Parkinson's Disease

  3. Performs instrumental testing including Modified Barium Swallow and Fiberoptic Endoscopic Evaluation of Swallow Studies

  4. Delegates and supervises patient related tasks to assistants, aides, and support personnel. Supervises students as assigned.

  5. Supports excellence of the organization and customer satisfaction

  6. Contributes to the ongoing profession developments of self, department, and the network

  7. Collaborates with other disciplines to provide coordinated interdisciplinary patient care

  8. Manages time effectively and demonstrates willingness to work a variety of scheduling patterns within individual control hours.

  9. Contributes to peer review as required.

  10. Makes independent decisions within the scope of defined job description, level of knowledge, skills and abilities but seeks guidance as necessary for performance of duties.

  11. Meets or exceeds productivity and documentation requirements.

  12. Accepts and utilizes feedback in a constructive manner and demonstrates respect for others

in open and constructive communications.

  1. Accepts other duties as assigned. Demonstrates flexibility when aspects of a task change.

ASHA Certificate of Clinical Competence or candidacy for ASHA Certificate Competence required

Credentials:

Current license to practice in Connecticut: Successful completion of minimum educational requirements for licensure including all clinical practice and comprehensive examinations. Temporary licensure may be considered at the discretion of the department manager.
