We Are Hiring
Casa Scalabrini, part of Ascension Living, is looking for caring and compassionate Social Services Specialist to join the team! This gorgeous skilled nursing and rehabilitation community is ideally located close to I-294 in Northlake, and offers high level nursing care, memory care and respite care.
What You Will Do
Assist clients by assessing their social services needs, solving problems, setting goals and obtaining required services.
Conduct initial intake procedures. Assess needs and eligibility of potential clients.
Participate in the initial assessment process, service plan of care development, and ongoing monitoring of each client's care.
Obtain extensive social history from client and/or family member upon admission to program.
Resolve client problems by clarifying the issue, determining the cause; selecting and explaining the best solution; expediting correction or adjustment; and following up to ensure resolution.
What You Will Need
Licenses / Certification / Registration:
- Social Worker, preferred
- High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
- 3 years of experience preferred.
