Carleton College Clinical Case Manager in Northfield, Minnesota
The Clinical Case Manager will provide students who access services at Student Health and Counseling (SHAC) triage and support with referrals and follow-up, including, but not limited to, students who present in psychosocial crisis or at elevated risk to themselves or others. Collaborate with health-care providers, staff in the Division of Student Life, faculty, academic advisors, chaplains, parents/families, community providers, and others to sustain a robust student referral/support network. This is a 10-month position, working mid-August through Mid-June with 8 weeks off in the summer. This is an in-person position. Typical working hours are Monday - Friday 8am - 5pm.
