Josselyn RN Case Manager in Northfield, Illinois

Job Summary

The RN Case Manager will work with psychiatric services team to coordinate and advocate for our patients' care. The RN Case Manager will be required to review patients' charts, and to meet with patients, providers, and other team members to establish the most suitable course of action. The RN Case Manager will be responsible to oversee the medication line, complete Prior Authorizations (PAs), send non-controlled medication refills on behalf of psychiatric providers, consult with the providers and/or Psychiatric Services Manager when appropriate.

Essential Responsibilities and Metrics

  • Provides high-quality service and maintains 50% of direct service hours per month

  • Oversee the medication line, address all voicemails within 24-48 hours of receipt

  • Complete Prior Authorizations

  • Send Non-Controlled Medication Refills on behalf of providers

  • Complete Medication Authorization Forms

  • Address all incoming faxed and electronic refill requests from pharmacy

  • Complete Service Ticket/Progress Note within three working days of day of service

  • Perform routine procedures (blood pressure measurements, administering injections etc.)

  • Assist with the maintenance of the Medication Closet

  • Assist with the management of the Sample Drug inventory to include: o Logs samples in

  • Manages sample supply

  • Facilitates communication between Josselyn medication providers and drug reps

  • Discards expired samples

  • Facilitate client referrals for additional services consistent with assessed needs physician recommendation and/or client desired outcomes

  • Provide brief crisis intervention, stabilization services and supportive counseling to clients

  • Provide face-to-face mental health assessments

  • Refer client to Psychiatric Services Manager, RN when appropriate

  • Regularly consult with psychiatric providers and/or Psychiatric Services Manager, RN on specific client related clinical issues

  • Under direction of psychiatric providers, RN case manager will provide patients medication education print outs that are focused on understanding illness, symptom management and medication compliance

  • Other duties as assigned by the Psychiatric Services Manager, RN

  • Assumes leadership role either by own initiative or when asked by the Psychiatric Services Manager, RN

  • Develop a cohesive and strong team-oriented relationship with physicians, psychiatric services case managers and other healthcare professionals; evaluate and report on patient health outcomes and work in partnership with interdisciplinary healthcare team to facilitate the best inpatient care

Qualifications

  • Bachelor's Degree in Nursing Required. In lieu of a Bachelor's Degree, an Associate Degree in Nursing with 5 years supervised clinical experience in mental health or human services would suffice.

  • Current licensure as a Registered Nurse in Illinois required

  • Minimum three years experience as a psychiatric nurse, providing clinical services and medical case coordination, providing services to people with serious mental illness, preferably in a community mental health setting

  • Knowledge and experience in leadership and supervisory roles preferred

  • Ability to work as part of a team of professionals

  • Bilingual (English and Spanish) preferred
