Are you an experienced Home Care RN who is passionate about providing top-notch care to clients in the comfort of their own homes? Griswold Mercer and Middlesex Counties is currently looking for someone like you to join our team! As the Home Care RN, you will be responsible for preparing clients initial plan of care, reviewing case notes every 30 days, completing 60-day home visits, supervising CHHA’s, including competency and skills checklist during orientation. Our clients live in Middlesex, Mercer and parts of Somerset Counties RN are required to maintain our Accreditation with CHAP and New Jersey license with the Board of Consumer Affairs What you can expect from us: • $50/75 per hour • Healthy, Inclusive, Transparent, Supportive and Flexible Work Environment • Mileage reimbursement • Opportunities for On-Going Trainings and Career Advancement Opportunities • Robust Employee Recognition, Engagement, Appreciation and Retention Programs What we will expect from you: • Ensure the delivery of high-quality care and services that meet the needs of our clients and their families • Provide guidance to ensure compliance with state and federal regulations • Provide ongoing education and training to the care team to enhance their skills and knowledge • Maintain accurate client and caregiver logs • Collaborate with other members of the care team, including physicians, therapists, social workers, and other healthcare professionals at client’s request • Must successfully pass a criminal background and a minimum of 4 references (2 personal and 2 supervisory)   You will be successful in the role if you: • The RN must be a registered nurse (RN) in good standing holding a BS degree in Nursing and 2 years combined public health nursing and progressive professional responsibilities in public health nursing OR an RN in good standing having 3 years combined public health nursing and progressive professional responsibilities in public health nursing • Experience in working with seniors and non-medical home care is a plus • Strong knowledge of state and federal regulations governing unskilled home health care • Excellent communication, interpersonal, and leadership skills • Ability to work independently and as part of a team • Strong organizational and time management skills Requirements * NJ RN license * Malpractice Insurance * Dependable automobile * A warm and friendly smile!
