Support individuals within the ABI/MFP program who live in their own home, assisting with personal care, activities of daily living, individual support, companionship, homemaker, and community habilitation. Duties include:

  • Develop rapport with individuals supported and foster their personal growth
  • Monitor and report consumer\'s progress, challenges and status
  • Work individually with any program participants, which may include assistance/supervision with mobility, recreational activities or other needs.

Qualifications:

  • 18 years of age or older
  • Valid US driver\'s license and ready access to a vehicle
  • HS Diploma/GED or work/life experience providing care
