The Outreach Coordinator shall provide assistance to known veterans residing in their assigned service area of Vermont as well as utilize all outreach techniques to connect with veterans who are not enrolled in the VA system and reside in Vermont.

The Outreach Coordinator will identify and assist those veterans who do not readily identify themselves as veterans due to lack of knowledge or education of the services and benefits available to them. The Outreach Coordinator shall initiate Government and/or Veteran approved remedies, manage and follow-up on all outstanding actions related to assisting Veterans with the medical enrollment process for the Veterans Affairs Medical Centers and other community resources providing assistance to veterans.

The Outreach Coordinator shall provide liaison services between Veterans, their family members and the Veterans Administration Medical System (VA Medical System) upon request by the Veteran and family members residing in both rural and metro counties of Vermont regarding medical and/or benefit issues and claims.

The Outreach Coordinator will setup and attend Outreach events provide the opportunity to bring community and unit awareness of the VVO program to all rural and metro counties served and to develop relationships with military/community service providers and Veterans.

The Outreach Coordinator will provide peer to peer support to the veterans and families residing in Vermont. Peer to peer support defined as people with similar life experiences who can provide support to those moving through difficult life situations. Peer support can combat the isolation and fear veterans exacerbated by surroundings that are rural in nature.

Maintain strict confidentiality of all information related to Veterans. Health Insurance Portability and Accountability Act (HIPAA) Privacy Regulations will be strictly followed to ensure confidentiality and right to privacy is maintained. Contractor shall not maintain any medical records on any Veteran.

Coordinates and participates in geographical county/community Outreach events. Outreach events may include unit drill assemblies, meetings or events held at Veterans organizations or events scheduled by community organizations that provide services of benefit to Veterans and their families.

Provide follow-up contact on referrals and as needed, provide additional assistance and referrals. Conduct after referral surveys with Veterans and provide a survey regarding success of those services.

Assist veterans in navigating the Veterans Health Administration and the Veterans Benefits Administration process to include, but not limited to, walk-ins referrals, referrals from OSFPD/Community and Medical partners, disability and benefit processing systems, enrollment process and VA policies effectively.

Use generationally (Silent, Baby Boomer, Generation X, Millennial) appropriate communication methods to present information to Veterans.

Provide veterans with program materials and information related to a variety of assistance programs.

Conduct briefs, presentations, educational briefings on the VVO Program and the services it provides.

Be well versed and thoroughly familiar with all programs, to include emergency services available to provide quality referrals to support the needs of the Veterans.

The Outreach Coordinator shall attend specialized training courses, seminars and meetings to maintain awareness of information and resources supporting initiatives for Veterans, have sufficient knowledge and training in areas of military sexual trauma and sexual harassment processes as well as resiliency to provide referrals as needed

Complete online HIPAA training annually.

Develop and provide informational briefings to designated VTNG Military comman ers to increase their knowledge and understanding of veterans benefits, veterans service organizations and community partners who can assist military members is required within this contract.

The Outreach Coordinator will monitor the toll-free veterans help line and answer incoming calls on a rotational basis as assigned by the Program Manager. Upon receipt of a call, the coordinator will follow established guidelines to ensure the caller receives assistance as soon as possible after the call arrives. These duties will periodically occur during night shifts and on weekends.
