Position Overview: Responsible for supervising the personal care, companion and skilled

nursing services provided by company’s direct care staff; coordinating clinical services, and

ensuring quality of care of clients in assigned area. Duties include performing in-the-home

assessments to determine support and clinical needs, conducting regularly scheduled

supervisory visits of clients, documenting visits, and orientation and training of caregivers as

needed. Performance of skilled tasks may be required.

Responsibilities:

  1. Ensures that initial assessment and reassessment (supervisory visits) are performed timely as

required by rules and regulations.

  1. Supervises and evaluates the reassessment (within ten days) and skilled work performed by

LPNs as defined by Company policy.

  1. Ensures that client care issues, changes and/or updates are documented and communicated to

branch staff, direct care staff, case management and family.

  1. Reviews HRST and other documentation as required for DD cases, meeting with branch staff,

direct care staff, family and DD support team as necessary to understand and advise on clinical

issues.

  1. Provides or oversees the clinical orientation of new employees, including initial competency

evaluation and training of staff as needed

  1. Performs written Annual Competency Evaluations of all direct care skilled personnel (RNs and

LPNs) and non-skilled personnel (CNAs and PCAs). Participates in periodic conference with the

Branch Manager concerning direct care staff performance.

  1. Assesses and recommends solutions in areas of risk for clients, staff and organization

  2. Reviews and corrects documentation errors according to currently accepted standards of

medical practice.

  1. Ensures appropriate follow through and completion of documentation for incidents of clinical

nature.

  1. Provides PRN visits as necessary per Company policy and/or as directed by supervisor.

  2. Completes clinical audits as requested by supervisor.

  3. Ensures the compliant maintenance of TB testing supplies and proper disposal of syringes.

QUALIFICATIONS:

• Registered Nurse with valid license in state of GA

• Demonstrate high level of organization and communication

• At least 24 months of prior nursing experience, preferably within a multiple location environment

Knowledge and Skills:

• Excellent interpersonal, written, and verbal communication skills

• Strong analytical and problem-solving skills to assess situations and make accurate observations

in the workplace

• Knowledge of health administration regulations and standards, health information

management, and Medicaid, Waiver and Home health service lines

• Advanced knowledge of Microsoft Office Suite and aptitude to learn new applications

• Works accurately, efficiently, and independently

• Flexibility and ability to prioritize work with minimal supervision in a fast paced, high productive

environment

• Ability to maneuver multiple databases in the collection and submission of information

• Multi market or multi-site leadership experience (preferred)
