Posting Summary
Rutgers, The State University of New Jersey, is seeking a Community Coordinator for NJMS- Psychiatry. The primary purpose of the Community Coordinator, as a member of the Center of Excellence (COE) team, is to build relationships with individuals with opioid use disorder (OUD) in order to support the patient's journey towards recovery. The Community Coordinator will provide continuity of care by meeting patients in various clinical care settings, including during emergency department visits, addiction treatment appointments, medical appointments, social service appointments, as well as offering individual and group counseling. The Community Coordinator will also assist qualifying patients in accessing necessary public benefits to further support their wellbeing and recovery. They will also perform outreach by meeting with patients in the community. Individuals with personal experience with recovery from OUD or substance use disorders (SUD) are encouraged to apply.Among the key duties of this position are the following:
  • Engages patient upon diagnosis of OUD in a clinical setting (i.e. ED, medical clinic, social services office, etc.)
  • Responds with sensitivity to the cultural and socio-economic issues of the consumer.
  • Assess patient wellness needs and strengths. Assists patient in developing recovery goals and formalized recovery plan.
  • Provides educational information and resources as needed.
  • Links patient to appropriate clinical and social services, including but not limited to health insurance, housing, and disability benefits.
  • Provides reminder calls for all upcoming appointments as needed.

Minimum Education and Experience
  • Bachelor's degree or equivalent, or a high school diploma plus equivalent years of experience. Education and/or training may be substituted for the Bachelor's degree requirements.

WORK ENVIRONMENT:
Office environment and community site visits in difference neighborhoods. Moderate noise.
