Job Description

Job Overview: The Preferred Communities (PC) Intensive Case Management (ICM) Program, funded by the Office of Refugee Resettlement (ORR) offers individualized case management to the most vulnerable IRC clients. In FY22 with additional funds from ORR, the IRC’s ICM program created the Supplemental Case Management (SCM), a case management program that provides services to clients who exited their initial resettlement service period with additional need, but whose needs do not rise to the level of ICM eligibility. In FY23, the Virtual Supplemental Case Management (VSCM) Program was created to provide services to clients in need of case management who are outside the serviceable area of an IRC office or other resettlement agency and help local offices who need additional virtual support.

The Virtual Supplemental Case Management (VSCM) Coordinator will provide strengths-based, trauma-informed, and client-centered remote case management to clients located across the 27 IRC resettlement sites as well as clients located outside of the IRC catchment areas part of vulnerable groups: single-headed refugee households; survivors of domestic violence and gender-based violence; older refugees; youth and young adults without parents or permanent guardians; refugees experiencing mental health or psychological difficulties; refugees with physical disabilities or medical conditions; unanticipated refugee arrivals and secondary migrants; and refugees who identify as lesbian, gay, bisexual, transgender or queer (LGBTQ). The VSCM Coordinator will work closely with clients, colleagues, and external parties to develop service plans and coordinate services to meet individual client needs and preferences utilizing a broad range of financial, medical, social, vocational, and other resources in accordance with client eligibility and program requirements. The VSCM Coordinator reports to the Program Officer, Virtual Supplemental Case Management program in the Resettlement, Asylum, and Integration Department in the US.

Major Responsibilities:

Responsibilities include, but are not limited to:

  • Provide remote case management services to clients and perform crisis intervention, with guidance from supervisor as needed.

  • Develop individualized, SMART Self-sufficiency plans for each client enrolled in the VSCM

program.

  • Communicate proficiently and effectively with clients demonstrating knowledge and digital literacy.

  • Lead or co-lead efforts to coordinate services and service planning with other providers and agencies.

  • Collaborate with VSCM team, IRC colleagues, and community members to identify and develop partnerships to connect clients to nearby resources that meet client needs.

  • Provide education to guide clients in problem solving strategies and navigating various U.S. systems and to develop a supportive relationship.

  • Provide clients with extended orientation on a variety of topics such as health, navigating transportation systems, and navigating physical healthcare systems at local and state level(s).

  • Assist clients with requesting financial assistance when applicable and track the received

financial assistance within case files.

  • Maintain up-to-date documentation in client’s case file to ensure services are properly

documented throughout the client’s service period; document all services provided in IRC’s

database, Efforts to Outcomes (ETO).

  • Assess and make client referrals for other services as appropriate

  • May train and lead the activities of interns.

  • Other duties as assigned.

    Job Requirements:

  • Equivalent work experience or bachelor’s degree in social work or related field of study preferred.

  • Minimum 2 years case management experience that includes working with refugee and/or immigrant populations

    Preferred Experience & Skills:

  • Demonstrated knowledge of U.S. Refugee Resettlement and programs

  • Strong relationship building and ability to effectively build internal and external relationships

  • Ability to successfully work and communicate effectively in a multi-cultural environment.

  • Ability to work independently and collaboratively in a remote setting.

  • Ability to communicate proficiently and effectively with clients.

  • Self-starter with strong problem-solving and organization skills and ability to prioritize and manage time effectively.

  • Fluent in English, both spoken and written; proficiency in language(s) spoken by client group is desired.

  • Highly proficient in Microsoft Office applications (Word, Excel, Outlook); ability to use the internet and other digital tools for data entry, research, and problem-solving purposes.

  • Ability to work through change in a proactive and positive manner.
