Description

Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Center (Isaacs Center) are well-established Manhattan community-based agencies with a combined annual operating budget of approximately $63 million that support services to approximately 30,000 New Yorkers. Goddard Riverside and The Isaacs Center are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults throughout Manhattan and New York City. Pathways for Youth is an effort to help young adults reach age 24 prepared to be self-sustaining by providing a primary adult support to coach and assist in life planning and navigation over multiple years.

Purpose of Position: The Stanley M. Isaacs Neighborhood Center (Isaacs Center) is seeking an experienced, dynamic, and highly focused individual to serve within the larger scope of formal and informal learning of the Education and Workforce Program. The LMSW – Social Worker will work closely with participants ranging in age from 17+. They will carry their own case load of at least 40 individuals, with 20-25 meeting the qualification for bi-weekly counseling sessions. This person will work with their caseload with the multidisciplinary staff onsite to address barriers, formulate treatment plans and assist participants in meeting their goals. Weekly supervision will be provided by our onsite, LCSW – Clinical Director.

Schedule:Monday 9am-5pm; Tuesday through Thursday 11am pm to 7:00 pm; Friday 9am-5pm.

Salary:$58,500

Roles, Responsibilities, and Essential Duties

  • Complete biopsychosocial for all on case load and identify treatment plan w/SMART goals for those participants who qualify

  • Provide weekly/bi-weekly sessions for those clients who qualify for counseling services

  • Maintain notes, treatment plans for all participants on caseload in accordance with funders and agency policies

  • Identifies, predicts, and mitigates barriers to self-sufficiency including economics, education, and employment.

  • Works with participants to achieve educational, workforce and economic goals, enabling participants to access educational and employment services.

  • Determines social service needs, screens for benefits, assists with applications and makes referrals as necessary.

  • Provides individualized case management services and support to participants as needed.

  • Assists individuals with navigating complex systems including healthcare, social services, education, and financial systems.

  • Facilitates ongoing support to provide short-term interventions.

  • Tracks internal and external referrals.

  • Responds to participants’ needs for mental health, substance use, behavioral, housing, financial and other social services and identifies appropriate community supports as needed.

  • Conducts appropriate social and educational workshops to provide general and situational-specific information to participants.

  • Documents and tracks all interactions with participants through case notes and other tools according to timelines set by supervisor and/or funding specifications.

  • Manages the collection and distribution of program outcomes statistical data for reporting purposes, including adherence to program standards and government contract reporting.

  • Cultivates strong community-based partnerships with organizations that can serve as referral partners to meet the needs of participants.

  • Performs other duties as required.

    Qualifications/Educational Requirements

  • At least 2-3 years of case management, workforce development, educational counseling or other relevant experience is required.

  • Masters Degree in Social Work w/ current LMSW license in good standing in New York State

  • Strong writing, speaking, interpersonal and organizational skills.

  • Demonstrated experience providing case management, facilitating group work, managing data, and effectively coordinating entitlements, benefits, and community resource network, completing treatment plans and monitoring client progress.

  • Must value team building, be able to work to independently, and the understand how to actively engage the community to address complex issues.

    Knowledge, Skills and Abilities

  • Must be able to work with a diverse population.

  • Knowledgeable with Microsoft Office and databases, such as Salesforce and ASIST is a plus.

  • Must be able to effectively multitask and maintain a high level of customer service skill

  • Knowledge of EBT as it relates to Trauma Informed Care and interest in expanding their knowledge of other therapeutic interventions.
