Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.

But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.

Reporting to the Assistant Program Director, Connect to Care the Social Services Supervisor , C onnect to Car e, NYP supervises a team of Case Managers who u tiliz e Harm Reduction and Housing First approaches when respond ing to clients and facilitate connections to an array of services that can help them overcome their housing instability and address healthcare and other needs .

This is an overnight position and has a 35-hour work week.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Supervise Case Managers including scheduling, supervision of client cases, providing guidance and clinical direction, and ensuring timely and effective response to homeless conditions

  • Serve as a liaison with community partners and in close collaboration, follow up on homeless conditions, and community concerns

  • Develop strong collaborative relationships with external partners and service providers, NYPD, EMS, and other stakeholders

  • Oversee and participate in Code Blue/Red Alerts

  • Ensure documentation and reporting is accurately reported on time

  • Develop strong working relationships with clients and Breaking Ground coworkers, specifically colleagues in the Street to Home and Safe Haven programs

  • Attend onsite meetings with community partners

  • Perform other duties as assigned

    MINIMUM QUALIFICATIONS:

  • High School diploma or equivalent required; Master’s degree in related field preferred (e.g., Social Work, Public Health)

  • Experience with the street homeless population strongly preferred

  • Understanding of harm reduction required

  • Housing-based case management experience strongly preferred

  • S upervisory experience preferred

  • Ability to positively engage with clients who have significant mental health issues, current substance use, and often medical illness and cognitive impairment

  • Superb collaborative skills; Ability to work successfully with a wide range of internal and external stakeholders

  • Ability to communicate (verbally and written) with diverse populations and stakeholders

  • Excellent computer skills, including proficiency in Microsoft Word and Excel. Familiarity with Access-based databases and the ability to learn new programs are preferred

  • Bilingual Spanish/English is preferred

  • Preferred: Valid NY driver’s license with a driving record in good standing. If hired as a driver, maintaining a good driving record (as defined by the organization) is a condition of employment
