Description

TheHousing Coordinatoris responsible for the Supervision, review, and overall coordination of supportive services, home visits, social service referrals, entitlements management and housing stability management for the housing department. The Housing Coordinator is required to provide task supervision to the Housing staff as well as provide supportive services in both Spanish and English, based on client need. Responsible to ensure compliance with contract services and ensure all clients are engaged in health care and additional services based on client need.

Essential Job Functions

The following duties are mandatory requirements of the job:

  • Supervise and Maintain housing program census by managing intake and housing referrals.

  • Complete all onboarding processes and training for Housing staff.

  • Ensure the proper maintenance of client housing recordkeeping/archiving

  • Provide a range of case management services including but not limited to counseling, health care linkages, advocacy, entitlements management etc.

  • Complete all required paperwork by established deadlines. Paperwork work includes dashboard review deliverables report, DDAS and time and effort forms.

  • Provide coordination of care within GMHC and outside providers.

  • Review and ensure all contract deliverable services (home visits, advocacy, escorts, groups, counseling sessions etc.) are met for the HOPWA and HUD housing Programs.

  • Review and approve all data entered in all reporting systems which include eCOMPAS, TREAT, AWARDS and time and effort reports to meet contract mandates.

  • Ensure staff and clients have a strong understanding of the Terms and Conditions of Tenancy.

  • Assist Property Management Department with managing rent collection.

  • Attend and moderate case conferences with housing Case Managers, clients, and providers.

  • Attend all GMHC meetings and events as assigned.

  • All other duties as assigned.

Requirements

Education and Certification

  • Associates Degree, supervisory experience and at minimum 5 years’ experience working with clients with a history of housing related issues.

  • Bachelor’s degree preferred.

  • Fluent in English and Spanish preferred.

Special Skills and Knowledge

In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:

  • Knowledge of housing resources related to housing stability.

  • Extensive knowledge utilizing Microsoft Office Suite.

  • Ability to work within a fast – paced, data –driven, performance-based environment.

  • Exceptional written and oral communication skills with the ability to interface with a variety of internal and external colleagues.

  • Extensive experience working with individuals with HIV/AIDS, substance use issues and mental health needs.

  • Demonstrated ability to work effectively as part of interdisciplinary team.

Salary Description

$55,000 per year
