Speech Language Pathologist

Centers Plan for Healthy Living is expanding its footprint and we are seeking Speech Therapist to provide home-based speech therapy services.

Responsibilities:

  • Make home visits to clients.

  • Evaluates patients referred for home care services to determine appropriateness for speech therapy services.

  • Establishes a home care plan including assessment of the appropriateness of the requested services.

  • Provides rehabilitative services for speech and language disorders to meet established treatment goals.

  • Evaluate patients’ conditions for swallowing or cognitive deficits.

  • Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression.

  • Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient, family and other caregivers.

  • Keep accurate records of patient’s progress, goals and attendance.

  • Selects and use appropriate diagnostic and therapeutic techniques and materials.

  • Documents type of treatment and patient’s response on designated clinical progress notes.

    Qualifications:

  • Master's degree in Speech Therapy or a foreign degree equivalent as accepted by US Citizenship and Immigration Services or a Bachelor's Degree or foreign degree equivalent plus 2 years of progressive, post-baccalaureate experience.

  • Valid New York State license as a Speech Language Pathologist.

  • Excellent communication skills and the ability to interact well with diverse individuals.

  • Basic computer skills.

  • New graduates welcome

  • Able to function independently, with minimum supervision

  • Current Driver’s License, required insurance and car available for work
