The Home Care Nurse is a Registered Professional Nurse who administers skilled nursing care to patients requiring professional nursing service in the home. You will play a critical role in delivering top-notch care to our patients, promoting their well-being, and ensuring they receive the highest standard of medical attention.

Key Responsibilities:

Assess the healthcare needs of residents and develop individualized care plans.

Administer skilled nursing care as prescribed by physicians and ensure the execution of treatment plans.

Monitor and document resident's medical conditions, vital signs, and response to treatment.

Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to provide comprehensive care.

Educate residents and their families on health conditions, medications, and self-care techniques.

Ensure compliance with all state regulations and facility policies.

Utilize computer systems for accurate record-keeping and reporting.

Participate in staff meetings and training programs to enhance clinical knowledge and skills.

Qualifications:

NYS RN license.

2-4 years of clinical experience.

Computer literacy to effectively use electronic health records and other digital healthcare tools.

The salary range for this position is $95,000-$110,000/year
