The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.

Position Summary

The post-transplant Clinical Coordinator ll-RN position will focus clinical responsibilities on outpatient care after transplantation as well as care during the listing process. Additional responsibilities will include clinical care of patients in our cardiomyopathy, cardio-oncology, advanced Fontan care, hypertrophic cardiomyopathy, and cardio-genetics practices.

A successful candidate will be expected to work collaboratively with Physicians, NPs, Registered Nurses, Research Coordinators, other members of the multidisciplinary care team, and administrative support staff. They will be engaged in all aspects of patients' clinical care and work with other members of the care team to provide seamless care across all venues including inpatient and outpatient. They will be expected triage patient concerns, perform complex medication reconciliation, and support the full clinical responsibilities of the program. A large component of the role revolves around counseling and educating patients and their caregivers on disease processes and care management.

Responsibilities

  • Triage patients in clinic in collaboration with MD and or NP to ensure high-quality care of patients in the practice.
  • Provide health education to patients and families.
  • Review labs and perform complex medication reconciliation.
  • Collaborate with the interdisciplinary team on patients' clinical care to ensure smooth and effective transitions of care.
  • Assist with other relevant administrative tasks.
  • Multiple QAPI and education opportunities.
  • May participate in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience, and patient safety.
  • Performs related duties & responsibilities as assigned/requested.

Minimum Qualifications

  • Bachelor's degree or equivalent in education and experience required plus four years of related experience.

Preferred Qualifications

  • 1-3 years relevant experience.
  • BLS/PALS Certification.

Other Requirements

  • Current NYS RN License, maintain and renew when necessary. (RN's are required to have a NYS license to qualify for this position)
