Purpose and Functions Under the direction of the Middlesex Emergency Services Manager, the Middlesex PATH/DMHAS Covid-19 SOAR Specialist uses the SSI/SSDI Outreach, Access, and Recovery (SOAR) Model with homeless populations to assist in accessing benefits. Provides technical Assistance to Middlesex Coordinated Access Network (CAN) including support for action plan implementation, training observations with feedback, and assistance with tracking outcomes in order to document success and help access additional resources.

Duties and Responsibilities

  • Coordinate the SOAR application submission process through a comprehensive review of each application for accuracy and completeness.
  • Provide technical assistance regarding electronic submissions to the Social Security Administration and entering submission data into the federal database.
  • Act as liaison to Medical Records and other hospital departments, Disability Determination Services, Social Security district office and other relevant services when necessary to secure provision of documents.
  • Direct the appeals process as necessary.
  • Attend weekly and monthly agency, program, and network meetings.
  • Supports behavioral health provider organizations by ensuring best practices are followed with regards to SOAR implementation, monitoring and reporting at each organization.
  • Assists with increasing system capacity of representative payees for SSI/SSDI recipients.
  • Other related duties as assigned.

Qualifications

  • Bachelors Degree in related field with 3 years work experience in a professional environment.
  • Valid drivers license with a good driving history.
  • Excellent organizational and time management skills with a deadline-oriented mindset.
  • Strong written and oral communication skills employing the English language.
  • Proficient in Microsoft Office and other common desktop applications.
  • Knowledge of behavioral health service models.
  • Strong familiarity with Social Security Administration processes.
  • Ability to collaborate successfully with others.
  • Able to meet all documentation requirements.

Supervised by:Middlesex Emergency Services Manager

Salary:$19.23 per hourbased upon education and experience plus excellent benefits

Schedule:Monday thru Friday 9:00 a.m. to 5:00 p.m. with some flexibility
