This network of services provides accessible mental health services within the primary care medical home, and at our regional mental health specialty hubs across San Diego and Southern Riverside county. Our program is focused on supporting early identification, health promotion and illness prevention, in collaboration with our primary care pediatric partners. We have a growing team of clinicians focused on providing total child health and mental wellness.

The IHT will conduct a brief model of mental health services, including diagnostic assessment, triage, psychotherapy, and multidisciplinary team treatment planning. As a generalist provider, the IHT will offer education, connection to resources, counseling, problem solving, brief therapy, and crisis intervention. The IHT will develop integrated care plans utilizing knowledge of mental health, age, developmental stages, and culture. This role works closely with physicians to provide holistic care, monitor patient progress, and achieve treatment goals to improve both mental and physical health.

The IHT may engage in teaching opportunities including workshops or presentations for youth, families, providers, and/or community members on topics related to mental health, integrated care, and wellness. As TMH programming evolves, opportunities for collaboration in research projects may become available, as well as opportunities for professional advancement.
