Elevance Health RN Specialty Home Infusion Per Diem - Paragon Healthcare in MONTGOMERY, Alabama

Registered Nurse, Specialty Home Infusion

Work Shift: Per Diem, 2 to 3 shifts a month and On-Call coverage.

Location: Field Based position. Visiting patients in their home. Ideal candidates must live within 50 miles or a 1 hour commute to the Elevance Health Office in Montgomery, AL.

The Registered Nurse, Specialty Home Infusion is responsible for coordinating and performing infusion nursing care for patients in a home setting.

How you will make an impact:

  • Administers skilled nursing care; participates in educating patients and caregivers with home medical management.

  • Provides leadership by working cooperatively to establish priorities and goals of patient care.

  • Conducts patient assessments, patient/caregiver teaching, development and revision of the plan of care, implementation of physician orders, monitoring patient response, response to emergency patient needs and psychosocial support.

  • Follows company policies and procedures as well as INS standards of practice, OSHA guidelines, State Nurse Practice Act and state board laws/guidelines.

  • Provides pre-discharge patient assessments in the hospital or clinic setting, when appropriate.

  • Completes all required patient care documentation and reports accurately. Submits all required patient care documentation and reports in a timely manner.

  • Communicates to supervisor, physicians and other health care providers, as appropriate, any changes in the patient’s condition.

  • May have on call responsibility as assigned to provide 24 hour availability and provides these services in accordance with established protocols.

  • Utilizes universal precaution when working in potentially hazardous situations (e.g. handling of blood and body fluids).

  • May participate in the orientation of other nurses as requested.

  • Provides educational programs or visits to referrals sources, as requested.

Minimum Requirements:

  • Requires AS in Nursing and minimum of 2 clinical experience or home health experience ; or any combination of education and experience, which would provide an equivalent background.

  • Current, unrestricted RN license in the applicable state is required. Multi-state licensure is required if this individual is providing services in multiple states.

  • For Paragon and Carelon Health business unit, satisfactory completion of a Tuberculosis test is a requirement for this position and Hepatitis B vaccine or signed waiver.

Preferred Skills, Capabilities and Experiences:

  • Experience with IV administration, access PICC lines

  • Experience conducting patient assessments/education.

  • Comfortable with local travel visiting patients in their homes.

  • Experience treating hemophilia patients.

  • BSN preferred.
