Milford Regional Medical Center Point of Entry/Emergency Department Case Manager in Milford, Massachusetts

Per Diem, 9a-9:30P, as needed weekends and occ/Holidays

Statement of Purpose: In collaboration with other members of the healthcare team, the Point of Entry/Emergency Department Case Manager (POE/ED CM) is accountable for coordination and facilitation of the management of care for ED patients, both inpatient & observation. The POE/ED CM is responsible for managing the patient flow through the ED through evaluation of incoming patients and assisting to direct the appropriate discharge disposition (Home, Post-acute Services, Inpatient or Observation) from the ED. This is done with an emphasis on early assessment of discharge needs as well as communication among the healthcare team members and the family/care givers. The POE/ED CM is responsible for collaborating with the admitting teams to ensure the correct initial admission status (INPT or OBS) at the time of admission using Interqual criteria.

Qualifications:

Education: Bachelor of Science in Nursing required.

Certification/License: Current Massachusetts licensure required. CCM preferred.

Experience: 3-5 years of hospital-based case management with emergency department experience preferred Minimum of three years of medical, surgical, ICU or ED nursing experience required Demonstrated ability to make sound judgments with knowledge of current medical care technology and ancillary supports. Previous utilization review experience preferred Must be assertive with excellent interpersonal skills necessary to interact effectively with all levels of professionals within the hospital and in the community. Analytical ability to interpret patient-related information; to evaluate appropriateness of continued stay and/or need for ancillary services.
