The UMHC – SCCC Social Services Department at UHEALTH/University of Miami, has an exciting opportunity for a Full Time Social Worker.

SUMMARY

The Social Worker, MSW provides social services to patients and their families, including counseling and support. Moreover, this position establishes a plan of care that fosters personal worth, spiritual well being, and human dignity in each patient.

CORE JOB FUNCTIONS

  1. Collaborates and facilitates patient discharge plans for those requiring post inpatient care.2. Reassesses plan of care according to the physical and emotional needs of patient.3. Performs patient assessment, including looking at illness, mental status, and family dynamics.4. Documents relevant data accurately and in compliance with laws and regulations.5. Links patients and their families to appropriate community resources.6. Serves as patient advocate and liaison to physicians, insurance companies, and community agencies.7. Helps the patient and their family to understand, accept, and follow medical recommendations.8. Provides clinical supervision and guidance to junior social workers.9. Adheres to University and unit-level policies and procedures and safeguards University assets.

CORE QUALIFICATIONS

Education:

  • Master’s Degree in Social Work

  • Person hired into this position is required to be licensed within three years of start date.

Experience:

  • Minimum 1 year of experience

  • Bilingual: English/Spanish

Certification and Licensing:

BLS Certified (within 3 months of hire)

Department Specific Functions

  • Able to provide systematic psychosocial screening and assessments regarding mental status and psychopathology and communicating pertinent information to the healthcare team.

  • On-call rotation for the facility to receive and facilitate after-hour calls.

  • Able to demonstrate the ability to advocate for psychosocial-spiritual concerns and ensure that care is safe, efficient, effective, person-centered, timely, and culturally congruent.

  • Documents appropriately and thoroughly in the electronic medical record.

  • Formulate, develop, and implement a plan utilizing appropriate social work interventions.

  • Collaborate with the health care team and involve the patient/family in the development and implementation of plans. Facilitate discharge planning across a multidisciplinary care continuum that includes interacting with consulting physicians, third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate discharge plan.

  • Works collaboratively with the multidisciplinary staff to design and implement treatment plans and discharge plans for the patient.

  • Be able to provide appropriate referrals depending on the needs of the patient.

  • Be able to assist with financial issues.

  • Facilitate relaxation exercises and assist patients and family members in achieving positive coping strategies.

  • Provide education and counseling to patients and families around issues related to adaptation to the patient's illness and/or life situation. May utilize crisis intervention, brief and long-term individual, group, and family therapies.

  • Develop and facilitate support groups or special functions.

  • Address and facilitate advance directives and able to assist patient/family with completion.

  • Able to practice with commitment to provide evidence-based care.

  • Address survivorship issues

  • Provide education to other health care professionals.

  • Participates in or consults on research/education programs.

  • May function as a resource to other staff as appropriate.

  • Perform other duties as assigned.
