Job Summary:

Acute Home Healthcare, Inc. is seeking compassionate and skilled Registered Nurses (RN) to join our home healthcare team, serving patients in the Metro Detroit area and nearby communities. This position offers flexible hours and immediate placement for the right candidates. As a home health RN, you will provide high-quality, individualized care to patients in their homes, helping them recover, manage chronic conditions, or maintain their independence.

Key Responsibilities:
  • Patient Assessments: Conduct initial and ongoing assessments of patients' health status, including physical, emotional, and environmental factors.
  • Care Planning: Develop and implement individualized care plans in collaboration with the healthcare team, patients, and their families.
  • Skilled Nursing Care: Provide skilled nursing services such as wound care, injections, IV therapy, medication administration, and monitoring vital signs.
  • Education and Support: Educate patients and caregivers on disease management, medication adherence, and lifestyle modifications to promote optimal health outcomes.
  • Monitoring and Evaluation: Regularly assess patients' progress and modify care plans as needed, reporting any significant changes to the physician or care coordinator.
  • Documentation: Maintain accurate and timely clinical records, ensuring compliance with regulatory standards.
  • Collaboration: Coordinate care with other healthcare providers, including physical therapists, occupational therapists, and home health aides.
  • Emergency Response: Recognize and respond appropriately to medical emergencies, ensuring patient safety at all times.
Qualifications:
  • License: Active, unrestricted Registered Nurse (RN) license in the state of Michigan.
  • Experience: Previous experience in home healthcare, hospice, or a clinical setting preferred but not required.
  • Education: Graduate of an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Certifications: Current CPR and First Aid certification required.
  • Skills: Strong clinical skills, excellent communication, and the ability to work independently in a home care environment.
  • Transportation: Reliable transportation required, as travel to patients' homes is necessary. Must have a valid driver's license and auto insurance.
  • Background Check: Successful completion of a background check and drug screening required.
