Amergis Social Worker in Melville, New York
Salary: $62,000 - $70,000
Hours: Monday - Wednesday: 2pm-10pm
Thursday - Friday: 9am – 5pm
Licenses accepted – LMSW,LMHC,LMFT,MHC – Limited Permit, MSW – Limited Permit, MFT – Limited Permit
The Licensed Clinical Social Worker collaborates with themulti-disciplinary team to provide input in the development of the plan of carefor those patients/clients requiring social work intervention. The Licensed Clinical Social Worker providescounseling and crisis intervention to patients/clients who have social oremotional problems in a variety of settings across the age span.
Essential Duties and Responsibilities:
+ + Facilitate various formats (e.g. individuals, groups, specialty groups) on time and in a professional, ethical and caring manner. Manage all aspects of a caseload from date of entry to date of discharge with special attention to appropriate levels of care and appropriate clinical interventions.
Provide various clinical or substance abuse services.
Conduct intakes and monitor individual, group, and family therapies.
Develop service plans and conduct reviews as needed throughout treatment.
Provide mental health assessments, counseling, referrals, and intervention services.
Complete and maintain documentation according to federal and state guidelines.
Other duties as assigned.
Ability to work effectively in collaboration with diverse groups of people.
Adheres to facility and Amergis policies and procedures at alltimes
Minimum Requirements:
Master’s degree in Social Work from a school of social workaccredited by the Council on Social Work Education required
Active license as an LCSW in state of practice required
Minimum of one year of social work experience preferred
Complies with all relevant professional standards ofpractice
Participation and completion of Amergis’ Competency programwhen applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
Computer proficiency required
Must be at least 18 years of age
