Job Details Under the guidance of the Chief Quality Officer and the Vice President of Quality Programs, the PI Clinical Quality Project Manager is responsible for leading projects that support the CHS Quality Strategic Plan initiatives. Initiatives include, but are not limited to, Reduction in Facility Acquired Infections, Improvement of Patient Experience, Optimization of Clinical Workflows in the Physician Practice Environment, Physician Strategy Engagements, etc. The individual in this position will be responsible for understanding quality program requirements as stipulated by the sponsoring regulatory agency, and abstracting clinical quality information from the medical record according to program criteria, when appropriate. In addition, the individual will serve as a liaison at the corporate level to ensure collaboration among entities. Position Responsibilities: Responsible for leading initiatives that support the enhancement of patient care and safety across the organization. Responsible for maintaining current knowledge on quality payment programs associated with all entities across the network. Works with clinical teams to ensure adherence to quality program requirements across the network. Works with providers and other clinical staff to develop clinical workflows that support patient safety initiatives and positive quality outcomes. Works with providers and other clinical staff to ensure clinical content of workflows are based upon current evidence-based guidelines. Responsible for reviewing medical records, and abstracting clinical data elements for quality measures according to published criteria. Responsible for entering quality measure data into appropriate quality databases to facilitate reporting. Completes chart review, data entry and other reporting requirements by deadlines as stipulated by the regulatory agency and/or CHS Leadership. Completes other duties/responsibilities as assigned by Department Leadership. In addition to quality coordinator responsibilities, this position will be required to conduct projects as a Project Manager Lead. Ensures the clear definition of project objectives and scope. Ensures the technical and operational feasibility of the project before its commencement. Identifies and includes all relevant stakeholders, as appropriate. Leads and facilitates all phases and techniques appropriate for a respective initiative including and not limited to: planning, design, build, testing, training, implementation, analysis and closure. Collects and documents project requirements, assists development of solutions and workflows. Ensures projects are delivered within the approved scope, schedule and budget. Develops, maintains or facilitates comprehensive project documentation as appropriate for a respective initiative. Manages change to project scope, schedule and budget. Performs risk and issue management addressing mitigation and resolutions accordingly. Works as liaison between IT and clinical disciplines to ensure accurate communication about requested information and that the most effective and efficient process efforts are achieved. Educates providers and other organizational departments on workflows and initiatives that support the project and improve member outcomes and satisfaction for assigned population(s). Manages data analytics within the scope of the project Interprets analytical outcomes from the data (including trends and variances) to support the quality improvement decision making process. Converts the results of data analysis into meaningful business information and conclusions. Identifies opportunities to integrate data from existing and new clinical/information systems Responsible for the analysis and review of data acquired from diverse medical record systems, clinical information systems and other sources. Ensures integrity of data and the data process. This includes: investigation and knowledge of data sources and the data process workflow; recommending data collection processes/workflows that support the efficiency and effectiveness of data that is collected; and auditing results to ensure data validity and reliability. Works extensively with other project team members to identify, analyze and recommend solutions related to collection and reporting of performance improvement data to streamline processes and improve data integrity. Performs ongoing analysis of gaps in clinical care and barriers to care; ensuring alignment of all interventions with regulatory agencies. Position Requirements: NYS licensed RN, LPN or Physician Assistant Associates, Bachelors or Masters degree preferred Six Sigma Green Belt certification a plus. Minimum of 5 years clinical experience required. Performance Improvement and Quality Management experience preferred. Prior experience in clinical quality measure data abstraction preferred. Must have knowledge of provider care principles, procedures, and practices. Demonstrates understanding of Hospital and Ambulatory/Practice-Based clinical environments and workflows. Experience in working with an Electronic Health Record. Proficiency in database, spreadsheet, and word processing software applications essential. Microsoft Office proficiency required. Familiar with Six Sigma, PDCA and/or other performance improvement methodologies Demonstrates the ability to effectively communicate with all levels of hospital and practice-based staff in a verbal and written manner Possesses keen analytical skills and demonstrates the ability to be very organized. Salary Range USD $130,000.00 - USD $140,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
