Health First Nurse Manager - Hospital at Home in Melbourne, Florida

POSITION SUMMARY:
To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by leading, planning, directing and assessing all unit functions for the Hospital at Home (HaH) program. Lead interdisciplinary team from patient assessment to discharge, including evaluation of patient care on a 24-hour basis on an assigned unit. Participate in the development of policies and procedures for the Department of Patient Care Services, implementation of Health First policies and objectives, budget preparation, quality improvement activities, and leading a cost-effective operation. Oversee the scheduling and assignments of associates and interdisciplinary team as appropriate to ensure quality patient care. Serve as clinical and professional role model to staff, contributing to their professional growth and development.

PRIMARY ACCOUNTABILITIES:
1. Manage day-to-day operation of the program, including appropriate identification of patients meeting the Hospital at Home (HaH) criteria, in coordination with the Emergency Department team, and as determined by need to best serve the patient
2. Conduct disciplinary conferences with individuals not following hospital policy/procedure or whose actions necessitate corrective action. Document individual counseling and disciplinary actions with associates at all times.
3. Perform and comply with Health First’s (HF) managerial responsibilities related to recruitment, hiring, coaching, development and evaluation of staff. Utilize resources, processes, and system to complete managerial responsibilities and expectations. Utilize data and reports, and pre-established standards to coach, develop and evaluate staff.
4. Communicate policies and procedures to ensure compliance with Health First and governing expectations.
5. Facilitate and promote high quality, cost-effective outcomes, focused on the whole patient orientation and self-management decision support and to minimize any fragmentation of health care delivery.
6. Provide appropriate delegation and supervision of patient care activities; collaborate with the hospital’s Quality department to monitor patient outcomes, in an effort, to meet or exceed regulatory compliance.
7. Collaborate with the hospital’s Quality department to monitor outcomes and utilize quality software programs for data entry to meet regulatory requirements. Promote quality patient care and customer service/satisfaction, with safe, cost efficient measures, and a commitment to process improvement.
8. Ensure compliance with The Joint Commission and other organizational and regulatory agency standards with quality indicators and core measures. Insure accurate and timely records for all patient care to comply and meet regulatory requirements, quality and other measurable goals.
9. Communicate timely with all stakeholders, and provide updates during daily team huddles and with other members of the healthcare team; promote interdisciplinary plan of care
10.Serve as role model for all unit staff by setting an example of high standards in conduct, attitude, performance and patient/family relations
11.Responsible for Patient Experience as evidenced by positive patient satisfaction scores for specific unit
12.Assume 24-hour accountability for unit activities, and ensure schedules are completed on time and in compliance with approved staffing plan; flex own schedule as needed, including leading other duties and assignments as needed.

MINIMUM REQUIREMENTS:
Education: Bachelor’s in Nursing
Work Experience: Two (2) year Acute care experience; One (1) year managerial experience in related field
Licensure: Current Florida RN license
Certification: Current American Heart Association BLS Healthcare Provider; eligible Level II background screening
Knowledge/Skills/Abilities:
o Regulatory - Knowledge of TJC (The Joint Commission), Florida Nurse Practice Act and other applicable agency regulations
o Clinical Competency – IV, nursing assessment, medication administration and other clinical skills as needed/required
