UPMC Medication Transition Coord I (Medication Histories) in Mechanicsburg, Pennsylvania

The Medication Transition Coordinator I will support medication transitions for patients during hospital admissions and discharges as well as select outpatient visits with a goal of assuring patients have medications and requested products prior to leaving the hospital. This goal is accomplished by identifying new admissions, obtaining accurate medication histories, explaining the Meds-to-Beds program, and assuring requested products are affordable to the patient. The Medication Transition Coordinator I is responsible for logistically organizing and delivering requested medications and other products prior to discharge.

Responsibilities:

  • Demonstrates flexibility in work assignment and location. Consistently communicates in a courteous and professional manner when interacting with customers and coworkers. Respects confidentiality and established boundaries when providing information. Performs other duties as assigned by manager.

  • Obtain and document complete and accurate medication histories. Introduces the Meds-to-Bed program to patient. If the patient is interested, ensures the patient has given permission to accept services, are eligible for services, and have method of payment. If patient in enrolled in the Meds-to-Bed program, changes the pharmacy of choice in the electronic system. Collaborates with the clinical team to assist with patients post discharge medications and requested self-care products. Assists patients who have financial concerns by collaborating with case manager. Ensures insurance information, prescriptions and product orders are sent to the pharmacy. Delivers prescription medications and/or products to patients bedside once fulfilled by Pharmacy. Collects method of payment and encourages counseling session with the Pharmacist. Demonstrates compliance with company policies and procedures. Adheres to HIPAA Privacy Standards and Requirements. Maintains working knowledge of specified equipment, services, and supplies available to transitioning patients. Completes annual requirements required by UPMC.

  • Understands how care is provided in a health care system and the roles of various providers and health care professionals. Comfortable working autonomously and in a self-guided role. Must be flexible and adaptable. Ability to assess and anticipate the needs of the patient and his/her environment. Effective ability to communicate including verbal, non-verbal, written, and listening skills. Ability to demonstrate professionalism, strong organizational skills, initiative, and an ability to work in a fast paced, rapidly changing environment. Must have sound judgment and decision-making ability. Comfortable working with patients, physicians, nurses, pharmacists and case managers. Experience working with computerized systems including database maintenance/management and email

  • Must possess the knowledge, skill, physical ability, and mental capacity to perform the typical duties and responsibilities enumerated below or implicit therein.

  • High school diploma or equivalent.

  • One year experience in a patient-related healthcare role. Education requirement (one of the following is required):o College Degree (minimally an Associate's degree) in a science or healthcare field with 1 year of experience in a healthcare role.

  • College Degree (minimally an Associate's degree) in a non-healthcare related field with 2 years of experience in a healthcare role.

  • Graduate of an accredited school of Practical Nursing with current Pennsylvania license.

  • Completion of 2 years of nursing or pharmacy education.

  • Certified pharmacy technician with 4 years experience as a pharmacy technician and 6 months experience as a medication history technician

  • Willingly reports, as assigned, to any facility that utilizes UPMC pharmacy services. Successfully completes all assigned annual competencies. Licensure, Certifications, and Clearances:
