Outreach Care Specialist II

Location : This position will work remotely. The ideal candidate will live within 50 miles of one of our PulsePoint locations.

*Hours: Monday-Friday 8:00 - 5:00 PM ES

The Outreach Care Specialist II is responsible for ensuring that appropriate member treatment plans are followed on moderately complex cases and for proactively identifying ways to improve the health of our members and meet quality goals.

How you will make an impact:

  • Coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs.

  • Telephonic outreach to members who have been discharged from an acute hospital.

  • Computer knowledge to utilize documentation programs and other applications as needed.

  • Refers members to digital programs.

  • Makes referrals to case management per case management criteria.

Minimum Requirements:

  • Requires a high school diploma or equivalent.

  • Requires a minimum of 3 years related experience; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities, and Experiences:

  • Certified nurse assistant/certified medical assistant or BS/BA degree in a related field is strongly preferred.

  • Bilingual candidates are strongly preferred.

  • Member/patient-facing experience preferred.

  • Medicare knowledge experience preferred.

  • Telephonic member-facing experience preferred.

  • Previous experience using various computer applications is preferred.
