JOB REQUIREMENTS: Personal Care Worker (PCW) will be trained in the provision of personal care services and in each skill that the PCW is assigned, which can include but not limited to: assistance with bathing; toileting, including use of bedpan, urinal, commode or toilet; dressing and undressing; mobility and ambulating, including use of walker, cane and crutches; transfers, including bed to chair or wheelchair and reverse; changing of consumer(s) bed; laundering of bed linens and consumer(s) personal clothing; skin care (excluding wound care); care of eyeglasses and hearing aids; light cleaning in essential areas of home during personal care activities; meal preparation and meal serving. Promptly reporting changes in the consumer\'s condition to the R.N. Case Manager. Completion of all time sheets and documentation necessary to maintain program services in a thorough, accurate, and timely manner. Maintaining confidentiality of consumer\'s information.
