JOB SUMMARY:

The Coordinator is responsible for coordinating HHA, LPN and RN placement to the clients and clerical functions. The Coordinator interacts with family members and clients alike and ensures their care needs are being met. He/she functions under the direction of the Administrator.

DUTIES AND RESPONSIBILITIES:

  • Understands software program HHAX and is responsible for daily attendance, prebilling and running reports.

  • Answers telephone, records and relays messages to appropriate personnel.

  • Manages telephone calls in a supportive, reassuring manner for patients, families, physicians and others.

  • Coordinates placement of services between clients and the paraprofessional staff.

  • Maintains and documents communication with paraprofessionals in all appropriate places as trained by supervisor

  • Maintains a schedule of all services per month.

  • Verifies billed services against requested services.

  • Assists with screening of home health aides.

  • Maintains patient confidentiality and privacy with dignity and respect.

  • Participates in patient case conferences, in-service programs and orientation.

  • Communicates with the supervisor regarding scheduling, availability, attendance, and assignment.

  • Participates in quarterly audits as needed

  • Other tasks as assigned by direct report.

    EDUCATION AND EXPERIENCE:

  • High school graduate.

  • HHAX and Microsoft Windows

  • Accurate office machine skills.

  • Good organizational and communication skills.

  • Mature individual, able to handle confidential aspects of the job.

SCHEDULE:

  • Monday - Friday; 9:00 AM - 5:00 PM
