Responsibilities Job Description

The Structural Heart Program RN provides a deliberate patient and family centered partnership with the healthcare team that facilitates a seamless patient experience between the Inpatient and Outpatient settings. The Structural Heart Program RN works to maximize the patient's health outcomes and the patient experience by identifying patient and family needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits. Works to develop and support a coordinated plan of care between visits to promote a smooth transition for patients and family across the healthcare continuum that instills a sense of trust and patient advocacy.

The Structural Heart Program RN duties Include:

  • Provides a deliberate patient and family centered partnership with the interdisciplinary Structural Heart team to facilitate a seamless patient experience between the Inpatient and Outpatient settings

  • Leads a team approach and serves as a single point of contact by facilitating and overseeing a subspecialty within the Structural Heart Program (LAAC, TAVR, TMVR)

  • Develops a patient centered care plan across the continuum: referral, consult, preop, post op, discharge, follow up

  • Reviews required test results and verifies that all necessary tests are completed and meet registry requirements for planned procedures

  • Reviews case summary and procedure planning at structural heart conferences

  • Completes all necessary frailty assessments when applicable to comply with the national registry database

  • Provides patient/family education during clinic consult with structural heart team

  • Provides patient/family education during preoperative visit including medication instructions, skin preparation and discharge goals

  • Provides patient/family with discharge education and follow up plan for continued care

  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards for catheter based structural heart procedures

  • Team approach working with other structural heart RN coordinators and actively participates in creating and implementing programmatic improvements and efficiencies.

  • Works to maintain primary care/referring physician involvement through ongoing communication and outreach education

  • Waitlist management and ongoing triage of critical patients

  • Supports care provided to patient in transition of inpatient and outpatient settings through shared communication of plan of care and patient/family needs

  • Ability to utilize technology to upload pre-procedure cardiac images to device vendors for case planning

  • Ensures that appropriate authorization for procedures/devices and medications have been obtained prior to services needed

Qualifications

Required

Education

  • Bachelor’s Degree in Nursing or 3-5 years of ICU experience

Experience

  • Proven Experience with an EMR

  • Excellent assessment and care coordination Understanding or willing to learn important criteria (inclusion and exclusion) required for surgical or percutaneous valve therapy.

  • Understanding basic CTA requirements/risks/results and TEE requirements/risks/results prior to either surgical or percutaneous valve therapy.

License/Certification/Registration

  • Basic Life Support BLS

  • Registered Nurse MN (RN)

Experience

  • 1 year of ICU experience preferred.
