Specifically, we are looking for people who could become part of the Mobile Health Program. We have teams led by an RN and include one or more medical assistants, outreach workers, social workers and providers (doctor, PA or NP). We need a someone who is willing to learn new ways of delivering care, adventurous enough to work in non-traditional settings and can be flexible with time and care delivery methods:

Mobile Clinic: We take vehicles converted into one exam room mobile clinics to various locations. Services include vaccinations, testing, health screenings, chronic disease management and preventive care. We serve all ages and various communities including agricultural worker housing, neighborhoods, schools, etc. Our current geographic priorities are San Joaquin and Stanislaus counties.

Street Medicine: On foot or with the mobile clinic, we provide health and social services to our patients who are unhoused (homeless). We help them navigate systems with the goal of increasing their capacity to manage their health, access medical care and avoid a medical crisis that requires an emergency room visit or hospitalization. We walk through homeless encampments, shelters and other settings. We also work with other teams within SJ Health and coordinate efforts with local health or social service providers. Our schedules tend to change as we focus on meeting the needs of the people we serve.

Minimum Qualifications:

BLS (Basic Life Support) : Familiarity with life-saving techniques and emergency response, DEA (Drug Enforcement Administration) : Authorization to prescribe and handle controlled substances. Candidates must hold a valid California (CA) license , Acceptance of candidates willing to apply for licensure in California is also considered. Previous experience in primary care, preventative care, and chronic disease management is required as well as familiarity with urgent care services , including health screenings, vaccinations, and disease management. We are seeking a compassionate provider who can connect with the underserved population .
