JOB REQUIREMENTS: Manitowoc County is seeking a qualified Public Health Nurse. This position provides population focused community-oriented nursing care based on the core functions of public health and: Performs surveillance, investigation, prevention and control of communicable and chronic diseases. Identifies population groups, families and individuals at high risk of illness, injury, disability, or premature death. Provides leadership for community health improvement activities. Addresses community health needs by collecting, reviewing and assessing data to determine service and program needs. Develops intervention strategies aimed at promoting healthy environmental, social, and economic conditions for all populations. Collaborates with public and private organizations to address community health needs.
