Schoolcraft Memorial Hospital Social Worker in Manistique, Michigan

POSITION SUMMARY:

Provides social work services to patients, and their families, in both the HomeCare and Hospice programs. Will assess and treat emotional needs related to illness. Will participate as a member of multi-disciplinary team and may assist patients in treatment planning, discharge planning, community resource needs, advanced medical directives as well as performing various assessments/screens. Master's degree of social work required. Experience in HomeCare and Hospice preferred.

QUALIFICATIONS:

  • Masters degree in Social Work

  • One year experience in a health care setting desired

  • Current CPR certification

  • Able to provide own transportation to patient’s homes

  • Satisfactory references

  • Positive physical and mental health

  • Able to work beyond scheduled hours as needed

  • Basic knowledge of computer desired

    WORK CONDITIONS:

  • Requires driving/travel to other sites, including in inclement weather.

  • Occasional exposure to infectious and contagious entities.

  • Occasional exposure to unsafe/unsanitary conditions in patient homes.

  • General office environment.

  • General office equipment used.

    DUTIES & RESPONSIBILITIES:

  • Responds to referrals of social work services from Schoolcraft Memorial HomeCare & Hospice.

  • Assess psychosocial needs of patients and/or families referred for services by reviewing the medical record, interviewing the patient and/or family and consulting with other members of the interdisciplinary Schoolcraft Memorial HomeCare & Hospice team.

  • Develop psychosocial treatment plan based on patient need, reimbursement guidelines and regulations.

  • Participate in interdisciplinary case conferences to discuss severity of psychosocial problems impeding patient progress and promote understanding of significant psychosocial factors impacting on health problems.

  • Contact attending physician as indicated to inform of progress and/or receive orders to modify the treatment plan as indicated to address changes in patient status.

  • Cooperate with Schoolcraft Memorial HomeCare & Hospice and interdepartmental services, medical care and community resources in executing the patient/family plan of care.

  • Provide advisory and consultative services to Schoolcraft Memorial HomeCare & Hospice professional staff as indicated.

  • Provide formal and informal training regarding communication, conflict resolution, the emotional, social and environmental factors affecting the health and well-being of patients and their families.

  • Document the patient/family progress toward identified goals response to treatment plan and assessment findings in patient medical record adhering to documentation guidelines/protocols.

  • Complete other Schoolcraft Memorial HomeCare & Hospice records and reports in an accurate and timely manner.

  • Demonstrate ability to organize and manage patient referral activities.

  • Participate in and support the Schoolcraft Memorial HomeCare & Hospice Quality Improvement Program and Utilization Review activities.

  • Comply with quality improvement standards, policies and procedures of Schoolcraft Memorial HomeCare & Hospice.

  • Adhere to the NASW Code of Ethics and standards of the profession and Schoolcraft Memorial HomeCare and Hospice policies.

  • Participate in Schoolcraft Memorial HomeCare & Hospice meetings, activities, programs, in-services and continuing education classes.

  • Maintain and increase knowledge base and clinical competence through attendance and participation in other professional development opportunities.

  • Maintain state licensure.

  • Maintain current knowledge of community resources and establish sound working relationships with these resources.

  • Remain flexible to changes in Schoolcraft Memorial HomeCare & Hospice policy and procedure and in daily scheduling.

  • Interpret the role and responsibilities of the Schoolcraft Memorial HomeCare & Hospice Social Worker to others as indicated.

  • Maintaining regular work attendance and adhere to established visit frequency per individual treatment plan.

  • Communicate and interpret policies and procedures to members of the Schoolcraft Memorial HomeCare & Hospice team, patients and families.

  • Adhere to Schoolcraft Memorial HomeCare & Hospice policies and procedures.

  • Communicate with Schoolcraft Memorial HomeCare & Hospice management if policies and/or procedures are violated.

  • Contribute to effective client relations by assisting patients, families, co-workers and other health care members to resolve expressed concerns and by demonstrating a positive and helpful attitude.

  • Maintain patient confidentiality by adhering to the terms of Schoolcraft Memorial HomeCare & Hospice Human Resources policy concerning confidential information.

  • Demonstrate appropriateness in meeting objectives in age-specifics.

  • Contributes to organizational operations by performing other related duties as may be required.
