Schoolcraft Memorial Hospital Advanced Practice Provider Float in Manistique, Michigan

POSITION SUMMARY:

The Advanced Practice Provider obtains detailed history and develops patient care charts; performs physical examinations and evaluations on assigned patients within the RHC and Home; assesses and treats medical problems and provides direct patient care to a specified patient population. They may work in Redicare, family practice, on home visits, or assist with a specialty as assigned.

QUALIFICATIONS:

  • Graduate of an accredited advanced practice practitioner program. Board Certified.

  • NP or PA with a current license to practice in the State of Michigan.

  • Current DEA license.

  • BLS, ACLS and PALS certified.

  • Minimum 3 years Rural Health Clinic or similar Clinic experience preferred.

  • Excellent clinical skills.

    DUTIES & RESPONSIBILITIES:

  • Participates in the development, execution and periodic review of written policies governing the services the clinic furnishes.

  • Provides services in accordance with those policies.

  • Arranges for or refers patients to needed services that cannot be provided at the clinic.

  • Assures that adequate patient health records are maintained and transferred as required when patients are referred.

  • Participates with a physician in a periodic review of the patient’s health records.

  • Provides acute non-emergency care to walk-in patients.

  • When appropriate, provides non-critical emergency care.

  • Consults/communicates with family physicians regarding care of patients as necessary.

  • Makes referrals as necessary to family physicians for follow-up care.

  • Dictates or provides a progress note in a timely manner on all office visits and telephone conversations (related to patient care).

  • Dress in a professional manner.

  • Display good interpersonal skills with the patients.

  • Display good interpersonal skills with the staff.

  • Maintain patient confidentiality at all times.

  • Comply with the standards set forth for RHC designation.

  • Assure proper follow-up on all abnormal test and imaging results.

  • Maintain the necessary accreditation on required certifications.

  • Demonstrate appropriateness in meeting objectives in age-specifics.

  • Perform other duties as assigned.
