The New Jewish Home Admissions Evaluator - temp part time in Manhattan SNF - New York, New York

The Manhattan Division of The New Jewish Home is searching for a temp part time, 20 hours/week, Admissions Evaluator to join their team. Onsite location is at 120 West 106th Street, NY, NY 10025.

The duties and responsibility of the Admissions Evaluator are:

  1. Reviews PRI’s and all supporting clinical documentation for appropriateness to the facility.

    1. Serves as a liaison to hospitals through the metropolitan area, making field visits as needed and telephone consultations to confer with hospital staff and to seek out and evaluate potential applicants for admission to the facility.

    2. Completes all required clinical pre-admission assessment forms on all potential evaluated, gathering additional medical and social information as need for a thorough assessment.

    3. Begins the application process during field visits and via telephone by disseminating information about The New Jewish Home programs; may gather psychosocial and financial information to complete applications with applicants and/or applicants.

    4. Documents all applicant clinical and psycho-social assessments.

    5. Submits monthly statistics and reports as requested by the Director of Admissions.

    6. Contacts Managed Care Companies to obtain pre-authorization; reviews level requirements to obtain appropriate level prior to admissions.

    7. Conducts tours of the facilities with potential patients, families and or representatives.

    8. Interacts with HMO Case Managers prior to admissions, to review skilled needs and maximization of level.

    9. Weekend and Holiday admission coverage as needed.

    10. Performs other duties as required.

      JOB QUALIFICATIONS:

    11. RN, LPN or Certified Social worker registered in the State of New York.

    12. MDS experience and PRI certification would be a plus.

    13. Previous experience in post acute admissions processes

    14. Acute Care experience preferred.

    15. Bilingual is a plus.

      SPECIALIZED SKILLS AND COMPETENCIES :

    16. Responds politely and helpfully to telephone and in-person requests for service consultations.

    17. Excellent writing and clinical assessment skills.

    18. Good working relationships with staff and referral services.

    19. Ability to multi-task and work accurately in a fast paced environment.

    20. Knowledge of Electronic Medical Record, Microsoft outlook, Word, and Excel.

    21. Required to speak and write in an understandable manner.

    22. Bilingual (English/Spanish) a plus.

      OTHER SKILLS AND COMPETENCIES:

    23. Ability to relate to adult and geriatric populations in a manner that respects their needs and capabilities.

    24. Thorough knowledge and understanding of medical terminology, conditions and treatments relevant to adult and geriatric populations.

    25. Ability to make thorough and accurate bio-psychosocial assessments of adult and geriatric applicants in relation to the continuum of services provide by the facility.
