Description Lutheran Social Services of WI and Upper MI is currently seeking a Forensic Care Coordinator to serve Dane and Rock County Our team provides case management for those that have been convicted of crimes, are in prison and have Diagnostic and statistical manual (DSM) mental health diagnosis . This role collaborates closely with the Department of Corrections to help individuals released from the correctional institution reenter the community successfully. Will be required to meet clients in the prison system and in the community to perform assessments, investigations/screenings to determine entry and placement in the Forensic Opening Avenues for Reentry Success Program. This is a full time, benefit eligible and remote role! No weekends! At Lutheran Social Services of WI and Upper MI, the health and safety of our colleagues and clients is our top priority!
