JOB REQUIREMENTS: Performs comprehensive mental health and substance use assessments, and summaries on a timely basis. Works closely with a multidisciplinary team to develop and implement individualized treatment plans and patient care. Collaborates with behavioral health and primary care providers and/or community service case managers for integration of care. Completes required documentation of services ensuring that member charts meet state and federal guidelines. Coordinates monthly recovery team meetings with all members. Attends weekly individual and group supervision. Facilitates referrals to other needed services and/or levels of care, including coordination of care with all CSS providers. Advocates for the needs of members and their families within the healthcare system. Maintains accurate recordkeeping and submits all claims within two business days.
