SUMMARY OF DUTIES: Works as a member of a multi-disciplinary clinical/rehabilitation treatment team providing medical services tailored to the individual needs of individuals with mental illness, substance abuse, and developmental disabilities. Performs ongoing monitoring of medication samples; assists psychiatrist during clinic hours; completes necessary clerical follow-up; communicates with pharmacies; completes prior authorizations with insurance companies; and facilitates medical, symptom and medication monitoring groups and individual sessions with participants.ESSENTIAL DUTIES AND RESPONSIBILITIES:•Completes Health Assessments on all participants•Assists psychiatrists / prescribers during clinic hours. Reports concerns to psychiatrists when necessary•Counsels clients in support of Individualized Recovery Plan•Facilitates assigned medical, medication, or symptom monitoring program groups•Maintains inventory of medications. Monitors medication samples ( including storing, tracking, expired samples disposal)•Monitors and records client’s vital signs•Provides ordered injections. Administers prescribed medications with monitoring•Obtains prior authorization with insurance companies•Provides referrals to local Primary Medical Doctor and coordinates care•Performs follow- through on lab orders•Monitors follow-up appointments•Intervenes in crisis and emergency situations•Completes all clerical responsibilities•Participates in weekly group clinical supervision•Participates in weekly staff meetingsEDUCATION AND TRAINING:Qualified health professional with a NYS RN Certification with a minimum of two years experience in mental illness, substance abuse and/or developmental disabilities. Must be actively CPR/AED certified. Proficient/able to learn Electronic Health Record.LANGUAGE SKILLSMust be able to communicate with and serve the mental health population with a compassionate, friendly and professional manner. Able to communicate with and assist clinic psychiatrists effectively, speak effectively Mental Health Counselors, and possess group facilitation skills. Ability to read and interpret documents such as safety rules, operating instructions and procedure manuals. Ability to write routine reports and correspondence.PHYSICAL DEMANDS:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
