Trinity Health Livonia is a beautiful full-service, 304-bed acute care hospital, located in Livonia, MI, that provides comprehensive care, including a 24-hour emergency department, general medicine, inpatient and outpatient surgery, physical medicine and rehabilitation, intensive care unit, cancer, cardiology, geriatrics and birthing and women's health. SUMMARY:

The Clinical Nurse Specialist (CNS) functions as an advanced practice registered nurse (APRN) within three spheres of influence (i.e. direct patient care, nursing-nursing practice, and organization-system leadership) to integrate care across the continuum. Although each sphere possesses its own distinct focus, the three together are interrelated and contain elements that overlap. The primary goal of the CNS is to promote the continuous improvement of nursing care and patient outcomes.

Direct Patient Care

  • Appliesthenursingprocess at an expert level toassess,diagnose, andtreatthecomplexresponsesofindividuals, families, or groupsto actualorpotentialhealthproblems.

  • Conductscomprehensive, holisticwellness and illness assessmentsusing established or innovativeevidence-basedtechniques,tools, andmethods.

  • Identifies and prioritizesnursingcare needs for a select populationofpatients/families.

  • Collaborateswiththeinter-professional teamtointegratethenursingperspectiveinto a comprehensive planofcareforthepatient/family.

  • Provides advancednursingcaretoselectedpatients, families, or groups acrossthehealthcarecontinuum in collaborationwiththehealth care team.

  • Initiatesandplanscare conferencesorprogramsforindividual patientsorpopulationsofpatients.

  • Designs and evaluatesinnovativeeducationaltoolsorprograms forpatients,families, and groups.

  • Identifies, collects, and analyzes datathatserve as a basis for program designandoutcomemanagement.

  • Establishesmethodstoevaluateanddocument nursinginterventions.

  • Evaluatesthe impact ofnursing interventions on fiscal and humanresources.

  • Servesas anexpertresourcefor thelatestevidence supportingcost-effective and safe nursingpracticesin a populationoffocus.

Nurse/Nursing Practice

  • Collaborateswithotherstoresolveissuesrelatedtopatientcare,communication,policies,andresources.

  • Usingevidencedbased information, creates and revisesnursingpolicies, protocols, and procedurestoachievedesiredoutcomes.

  • Identifiesfacilitators and addressesbarriersthat affectpatientoutcomes.

  • Leadsclinical practice and quality improvement initiativesfor a unit or program.

  • Collaborates withnursestodevelop practiceenvironmentsthat support shared decision-making.

  • Assiststhe stafftoenhancecriticalthinking and clinical judgment.

  • Creates a nursing careenvironment that stimulates continuousself-learning,reflectivepractice,feeling of ownership, and demonstration of responsibility and accountability.

  • Collaborateswitheducationresources andothercolleaguesoncontent and operational designoforientation,clinical competency, and other clinical educationalprogram development.

  • Mentorsnursesto acquire new skills, developtheir careers, andeffectively incorporateevidenceintopractice.

  • Providesinput for staff evaluation.

  • Providesformal and informal education for nurses andotherhealthprofessionals and healthprofessional students.

  • Leads in theconduct andutilization of nursing/clinical research.

Organization/System Leadership

  • Consultswith other units andhealthcareprofessionalstoimprovecare.

  • Leads/assistsinstitutional groupstoenhancetheclinicalpracticeofnurses andimprovepatient outcomes.

  • Develops, pilots,evaluates, and incorporates innovative models of practice acrossthecontinuum of care.

  • Designs and evaluatesprograms andinitiatives thatarecongruentwiththeorganization’sstrategicplans,regulatory agency requirements,andnursing standards.

  • Participates asneeded in identification, selection, and evaluationof products andequipment.

  • Advancesnursing practice throughparticipation in professional organizations, publications, and presentations.

  • Advancesnursing practice throughparticipation in clinical informatics.

  • Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner.

  • Helps to identify problems, offer solutions, and participate in their resolution.

  • Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors.

  • Discusses patient and hospital information only among appropriate personnel in appropriately private places.

  • Behaves in accordance with the Code of Conduct, Service Excellence Standards, and the Mission, Vision and Values of Trinity Health.

  • Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.

OTHER FUNCTIONS AND RESPONSIBILITIES

REQUIREMENTS:

Education:

  • Graduatedegree(master's or doctorate) in clinicalnursing from a programthat isaccredited by a nationalnursing body and prepares graduatestopracticeasClinical Nurse Specialists i.e. trained in CNS core competencies in the three spheres of influence

Experience:

  • Three tofive yearsprogressively more responsiblerelatedworkexperience.

  • Clinicalknowledge in a specialty area. If hired in an areawith a substantiallydifferent clinicalknowledgeset,thecandidatewillbeexpectedtodemonstrate progression of clinicalexpertiseinthepractice area within 6 monthsofhire.

Certification/Licensure:

  • Current and valid RegisteredNurse license and Clinical Nurse Specialist license inthe State of Michigan

  • Certification as a CNS through an accredited nursing body e.g. American Nurses Credentialing Center or American Association Critical Care Nurses
