HMS collaborative reviewer

The Clinical Nurse Reviewer’s chief responsibility is to collect and submit reliable data on eligible cases as required for participation in the Michigan Hospital Medicine Safety (HMS) Consortium, a Blue Cross Blue Shield of Michigan-funded Collaborative Quality Initiative (CQI). This is accomplished through high-quality clinical screening, data compilation, documentation and entry into the HMS database of all eligible cases for the hospital. The Clinical Nurse Reviewer also works closely with the physician champion and appropriate hospital leadership to identify opportunities for clinical quality improvement and other special projects as may be identified.

PERFORMANCE DUTIES

Documenting Patient Care

  • Responsible for the identification of patients for inclusion in the program registry through the application of program inclusion/exclusion criteria.

  • Responsible for independently managing the case load and reporting data to HMS according to established standards and deadlines. Data components will be obtained through the effective utilization of the hospital medical record system.

  • Demonstrates applicability of the methodology and the reliability of definitions utilized by reviewers within the program.

  • Responsible for identifying areas for streamlining and process improvement in the data collection process.

  • Demonstrates appropriate utilization of resources necessary to obtain valid, reliable data for entry into the program.

Works Effectively with Patients, Families, and Staff

  • Establishes effective working relationships with members of the hospital community, especially staff in the Medicine, Nursing, Medical Records and the Information Systems Departments whose support is necessary for the management and success of the program.

  • Contacts patients and/or families via telephone and/or written communications post-discharge for the purpose of identifying possible clinical concerns.

  • Actively engages in HMS activities and identifies opportunities to improve the quality of care in the inpatient setting by working with physicians, nurses, other clinicians, and administrators to develop strategies to improve performance.

  • Serves as an educational resource for the HMS CQI for internal and external audiences by developing educational material and delivering presentations.

  • Maintains regular contact with the HMS physician champion and other members of the team at the hospital.

  • Attends pertinent hospital committee meetings as appropriate.

HMS Participation

  • Attends consortium-wide meetings and sub-group meetings as a representative of the hospital.

  • Prepares for and participates in site audits by the HMS Coordinating Center, resolves data discrepancies, and responds to HMS Coordinating Center requests in a timely fashion.

  • Participates in teleconferences with other HMS Clinical Nurse Reviewers.

  • Attends national conferences pertaining to the program as requested.
