Job Overview:

Provides direct clinical social work services to patients and their families in a multidisciplinary team approach. Completes an initial psycho-social assessment including social work treatment plan. Provides individual, family or group counseling, complex discharge planning, bio-ethics case consultation, referrals to community agencies. Ensures continuity of care for those patients with a serious illness and maximizes the patient s and family s abilities to meet the continuing care needs within their social and financial framework.

Qualifications:

Required:

  • Graduation from an accredited graduate school of Social Work with an MSW (Master s in Social work) degree

Certifications and Licenses Required:

  • State licensure required by the NJ State board of social work examiners.

  • Licensed social worker (LSW) or licensed clinical social worker (LCSW) depending upon years of social work experience and clinical course work.

Essential Functions:

  • Understands stages of growth and development to assess each patient s age specific needs to provide age specific treatment and care.

  • Documents interventions in patient s chart and submits monthly statistical information to the department.
