Position Description: A Medication Care Manager/Medication Passer is r esponsible for the accurate and timely distribution and documentation of medication for residents in accordance with each resident-s physician-s orders and applicable regulations.

Education/Training/ Experience: Preferred applicants are CNA, PCA, or HHA, in good standing with the State Department of Health. Two (2) years' experience in caring for a geriatric population preferred. CPR and First Aid Certification preferred. Moderate reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; auditory and visual skills; ability to bend, stoop, sit, stand, reach, push, pull, and lift items weighing 50 pounds or less; basic computer literacy; and satisfactory completion of post-offer, pre-employment physical, as required.
