Description

Eliot is seeking a candidate who has the ability to provide person-centered care to individuals with a wide variety of cognitive, physical, and socioemotional needs in order to support them in living a fulfilling and meaningful life. This includes providing support to individuals in the following areas: transportation/appointment needs; activities and exercise, maintaining a clean living environment, actualizing ISP goals, meal planning and meaningful socialization.

Eliot’s Developmental Disabilities Division is a CARF accredited, individual-centered continuum of services that provides innovative and high quality programming to adults with developmental disabilities. Our programs offer support and opportunities for individuals to enrich their lives, obtain housing and employment, develop personal skills and competencies, and enjoy a full range of community activities while developing a network of caring relationships.

Physical and Mental Demands: Frequent walking, standing, bending, reaching and lifting to

carry out job responsibilities. Ability to lift individuals when necessary due to physical

disabilities and/or inability to independently enter/exit vehicles. Must be able to safely operate a

motor vehicle as needed. Ability to work a flexible work schedule contingent upon program

needs. Ability to work effectively in a fast paced environment with periods of high stress. Ability

to physically carry out Crisis Prevention Intervention techniques.

Responsibilities:

  • Oversee all clinical and programmatic components and the coordination and implementation of all services. Supervise all aspects of general operations, ensuring optimal service delivery that is provided within contractual requirements.

  • Coordinate, create, implement and monitor program development, licensing, contract

    compliance, quality and outcomes.
  • Maintain client records, ensuring annual plans, quarterly reports and monthly notes are

    current and accurately reflect clients’ goals and needs. Maintain electronic data and paper

    documentation in accordance with Agency policies, funding source requirements and state

    and federal regulations.
  • Develop and monitor budgets, maintain and implement strategy designed to ensure fiscal integrity of programs.

  • Hire, train and supervise all program staff.

  • Monitor performance of staff to include conducting preliminary and annual performance

evaluations.

  • Conduct individual supervision with staff at least twice monthly and staff meetings monthly.Ensure that individuals are treated with dignity and respect in accordance with ECHS’ Human Rights policy.

  • Support individuals in exercising control over their lives and assist individuals in being fully

integrated in their communities by supporting them in the use of community resources,

encouraging them in establishing memberships within their communities, and by

encouraging appropriate risk-taking.

  • Participate in the development of Individual Support and Care Plans for all individuals;

ensuring individuals are involved to the greatest extent in identifying their own goals and

planning for their own futures. Ensure individuals’ goals reflect things important to them and

provide training in the areas identified to promote personal growth and accomplishment

  • Responsible for promoting and upholding the principles of Corporate Compliance and theECHS Corporate Compliance program. Adhere to all ECHS policies and proceduresincluding the Code of Conduct, Confidentiality, and HIPAA.

  • Act in an ethical, legal, and responsible manner, following all applicable ECHS policies,state/federal laws, regulations, and reporting requirements.
