Title: Hospice and Palliative Care Nurse Practitioner
What You’ll Do:
Assist family meetings to clarify goals of care, code status and, when appropriate, transition to hospice.
Assist with or facilitate family meetings to clarify goals of care, code status and when appropriate, transition to hospice.
Coordinate care among both internal and external clinicians and health care providers.
Provide on call services via phone for hospice patients, both in the community and at the hospice house.
Participate in care coordination meetings as needed, including mornings ICU rounds, St. Mary’s oncology, hospice house and palliative care interdisciplinary team meetings.
Act as a resource to staff, patients, family members and attending providers regarding pain and symptom management.
For patients at the Hospice house, serve as attending or covering provider and is responsible for ordering medications, treatments, and testing as appropriate.
Perform palliative care initial consultations and follow up visits in the community or acute care setting as requested by the primary care team.
Provide care based on current clinical practice guidelines and evidence-based practice.
What You’ll Bring:
Master’s degree in nursing and certification by the American Academy of Nurse Practitioners (AANP).
One year of recent clinical nursing experience required.
Current R.N. license in the State of Maine.
Current CPR Card.
Must have a reliable and insured automobile and have a current Maine driver’s license.
Independent malpractice insurance strongly advised.
