Title: Hospice and Palliative Care Nurse Practitioner

What You’ll Do:

  • Assist family meetings to clarify goals of care, code status and, when appropriate, transition to hospice.

  • Assist with or facilitate family meetings to clarify goals of care, code status and when appropriate, transition to hospice.

  • Coordinate care among both internal and external clinicians and health care providers.

  • Provide on call services via phone for hospice patients, both in the community and at the hospice house.

  • Participate in care coordination meetings as needed, including mornings ICU rounds, St. Mary’s oncology, hospice house and palliative care interdisciplinary team meetings.

  • Act as a resource to staff, patients, family members and attending providers regarding pain and symptom management.

  • For patients at the Hospice house, serve as attending or covering provider and is responsible for ordering medications, treatments, and testing as appropriate.

  • Perform palliative care initial consultations and follow up visits in the community or acute care setting as requested by the primary care team.

  • Provide care based on current clinical practice guidelines and evidence-based practice.

    What You’ll Bring:

  • Master’s degree in nursing and certification by the American Academy of Nurse Practitioners (AANP).

  • One year of recent clinical nursing experience required.

  • Current R.N. license in the State of Maine.

  • Current CPR Card.

  • Must have a reliable and insured automobile and have a current Maine driver’s license.

  • Independent malpractice insurance strongly advised.
