Overview

[[This dedicated and compassionate Program Manager will lead and operate our Healthcare for the Homeless Street Medicine Program. The Program Manager will be responsible for overseeing a team of medical assistants, AMET's (Advanced Medical Emergency Technicians), and community health workers. The primary focus of this position is to manage the street medicine program, including medication storage, collaborate with an existing healthcare for the homeless clinic, and ensure seamless patient referrals to primary care services.

Responsibilities

Improve health outcomes of homeless individuals through comprehensive street medicine initiatives.

Lead and manage the Healthcare for the Homeless Street Medicine Program, ensuring the delivery of high-quality medical care to homeless individuals within the community.

Supervise and support medical assistants, AMET's, and community health workers to optimize the effectiveness of the program.

Oversee the medication storage program, ensuring accurate inventory management, medication safety, and compliance with regulations.

Collaborate closely with the existing healthcare for the homeless clinic to ensure comprehensive care coordination and integration of services.

Conduct health assessments, administer treatments, and provide medical interventions to homeless individuals in various settings, including shelters, streets, and encampments.

Provide wound care, managing acute injuries and illnesses effectively in the field.

Administer immunizations and preventive health measures to safeguard the health of homeless individuals.

Build and maintain relationships with local community organizations, shelters, and agencies to enhance outreach efforts and program effectiveness.

Refer patients to primary care services, ensuring a continuum of care for ongoing medical needs.

Participate in staff training, development, and performance evaluations to maintain a competent and motivated team.
