Job Description

Pay Rate: $20/HR Shift Details 8:00am-4:00pm Monday-Friday POSITION SUMMARY:  The Program Specialist provides case management for a group of individuals served in Adult Day Programs. Duties include acting as a liaison with families, agencies, and Elwyn personnel and participating as a team member to advocate for the individual, implement the Individual Support Plan (ISP) as written, communicate change in need for individuals, supervise the implementation of the ISP and secure needed services. DUTIES AND RESPONSIBILITIES: Complete required documentation including but not limited to monthly reports, quarterly reports, annual assessments, and incident reports and maintain individual records for the program as well as for support coordinators. Ensure compliance with Department of Human Services, Chapter 2380, as follows by: coordinating and completing assessments and providing the assessment to team members participating in the development of the ISP, including annual updates and revisions of the ISP attending the ISP meetings fulfilling the role of plan lead, as applicable reviewing the ISP annual updates and revisions for content accuracy reporting content discrepancy to the Supports Coordinator (SC) or plan lead, as applicable, and plan team members implementing the ISP as written supervising, monitoring and evaluating services provided to the individual, as well as training staff of the ISP contents, which includes the support and supervision needs of the individual. reviewing, signing and dating the monthly documentation of an individual's participation and progress toward outcomes reporting a change related to the individual's needs to the SC or plan lead, as applicable, and plan team members. reviewing the ISP with the individual documenting the review of the ISP providing the documentation of the ISP review to the SC or plan lead, as applicable, and plan team members informing plan team members of the option to decline the ISP Review documentation. recommending a revision to a service or outcome in the ISP coordinating the services provided to an individual. coordinating the training of direct service workers in the content of health and safety needs relevant to each individual. developing and implementing provider service Communicate with team members, agencies and community members on behalf of the individual in order to meet his/her needs and regulatory requirements. Initiate contact with the support coordinator when needed to schedule an Individual Support Plan (ISP) meeting or when individual's needs have changed, so that the change can be reflected in the individual's record consistently. Participate in the development of the ISP including annual updates and revisions providing content input for accuracy and reporting content discrepancy to the SC or plan lead, and plan team members as applicable. Ensure the accuracy of the ISP and the implementation of the ISP as written. Review the ISP with the individual to ensure understanding and document the review. Review ISP with direct care staff to ensure understanding of the plan and that goals are being implemented correctly. Provide informal counseling to individuals as needed concerning workplace issues. Provide training to individuals and Elwyn staff members on required topics including but not limited to career path development, conflict resolution, how to handle workplace issues, self-advocacy, and work behaviors. Provide back-up for other program staff as needed. Participate in emergency response situations in accordance with established program protocol. Perform cardiopulmonary resuscitation (CPR), and crisis intervention techniques using agency-trained protocols. Perform other duties as assigned.

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS: Requires one of the following sets of education and experience: A master's degree or above in Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, Therapeutic Recreation or other human services field from an accredited college or university and 1 year of work experience working directly with persons with disabilities. A bachelor's degree in Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, Therapeutic Recreation or other human services field from an accredited college or university and 2 years of work experience working directly with persons with disabilities. An associate's degree or 60 credit hours in Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, Therapeutic Recreation or other human services field from an accredited college or university and 4 years of work experience working directly with persons with disabilities. Case management skills preferred. Excellent time management skills a must Knowledge of the program regulations (i.e. 2380) preferred. Demonstrated ability to work effectively as part of a team. Ability to perform CPR and crisis intervention techniques in accordance with agency trained protocol preferred. Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills. Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, and Outlook. Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record.
