SIGN ON BONUS: $250.00 (Limited Time Only) SUMMARY: Cares for person served who are New Opportunity Waver participants, who live independently by performing the following duties. *ESSENTIAL DUTIES AND RESPONSIBILITIES includes but not limited to the following. The following responsibilities shall adhere to the person served plan of care:*

  • All Home Care Waiver staff will create a caring environment for person served through good relationship building, proper planning of activities of daily living, assistance with cooking and a standard level of cleanliness, and attractiveness of the home.
  • All staff should be conscious of safety and prevention of potential hazards for person served.
  • S/he will likewise be responsible for the monitoring the health of person served during their assigned shift.
  • Assist and supervise in preparing all meals and snacks as per plan of care
  • Assists and monitors person served into and out of bed, automobile, or wheelchair, to lavatory, and up and down stairs.
  • Assists person served to bathe and groom self properly, and dress appropriately.
  • Accompanies person served outside home, serving as guide, companion, and aide, participates when necessary. Monitors person served at all times, knowing there location.
  • Maintains record of services performed and document apparent condition of person served.
  • Staff may administer medication as prescribed by physician's order. Any PRN medication must be approved by the supervisor on call before administering such medication.
  • Staff should always monitor person served who self-medicate. All medication whether administered by staff or person served should be documented on medication administration record form and progress notes.

  • Must provide transportation to persons served, if specified in that individual's plan of care.
  • Responsible for knowing and carrying out the established policies and procedures of the Lafayette Association for Retarded Citizens.
  • Required to attend ongoing in-service training.
  • Maintains person served rights and confidentiality at all times.
  • Performs other duties as assigned by CSS Director.

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE: Must have a high school diploma or general education degree (GED); and one to three months related experience and/or training; or equivalent combination of education and experience.
