Kodiak Area Native Association Population Health Coordinator in Kodiak, Alaska
Manages the planning, oversight and completion of projects related to population health initiatives; such as chronic disease management, cancer prevention, and immunizations. Responsible for providing administrative management of KANAs Health-related grants, supporting population health projects in coordination and partnership with the Grants Department.

- Prepares patient correspondence, including telephone and written correspondence, as well as manages mailings and other outreach communications. Assists with the creation of outreach materials and social media posts, in coordination with the KANA Communications Department, to target and educate patients about the options for chronic disease management, immunizations, and preventative health care.
- Manages and validates patient preventative screenings and chronic care disease registries in KANAs population health management software; including chart reviews of patient electronic health records for accuracy.
- Utilizes population health registries to organize patient outreach for those overdue for preventative screenings. Coordinates with Care Teams to schedule appropriate appointments or referrals.
- Works with the nursing department to provide administrative support to the Immunizations Program. This includes management of patient overdue lists; coordination of immunization needs in Village Health Clinics; and tracking patient declinations.
- Ensures outside records from referrals and screenings are obtained and properly entered into the patients electronic health record.
- Plans patient outreach activities to include coordination with village community health providers and village health fairs, such as FIT campaigns and Self-Management Blood Pressure Program clinics.
- Coordinates project activities as assigned and collaborate with other programs and departments staff.
- Prepares and completes associated grant reporting requirements, to include work-flows and processes for regular data collection and monitoring of programs, in coordination with KANAs Grant Department.
- Meets with regulatory agencies for grants regarding improvement activities and for grant reporting purposes.
- Participates in grant technical training as assigned and keeps abreast of changing grant standards, proposal guidelines and compliance requirements.
- Participates in KANAs quality improvement processes by working closely with the Quality Team to coordinate improvement projects as applicable for population health initiatives.
